Although there is some understanding of the mechanism of migraine, its precise underlying causes are unknown. It it is believed that initally their is a neuronal trigger in the brainstem which connects to blood vessels of the scalp and brain producing the phenomenology. Migraine is a genetic disorder with at least 7 genes contributing to the events. A further discussion of pathophysiology is in my full chapter.
